Case Management

At ASPPIRE of Mid-Michigan, our Case Management services provide personalized, holistic support to help adults with disabilities achieve their goals and thrive. From your first intake meeting to ongoing progress updates, our team works closely with you, your support system, and community resources to create a customized plan that addresses your unique needs. Whether it’s connecting you to services, fostering independence, or guiding you through life transitions, we’re here to ensure lasting success and a clear path forward.

How it Works.

Intake

  • Step 2: You and your chosen support system will participate in a Holistic Life Planning intake. This will be a 2-hour meeting to learn all about you and where we can support you!
  • Step 3: Your case manager will create a set of plans tailored to your specific needs, financial situation, and goals that you will choose from.

Picking a Plan

  • Each plan varies in direct contact with your case manager, pricing, and the services that are provided.
  • You, your case manager, and your designated support team will work together to discuss how the services will be carried out, when they will begin, and how often updates are desired.

Progressing

  • Initially, several goals will be set to help ensure your success and happiness. At ASPPIRE, we understand that your needs will evolve over time. Our goal is to adapt to these changes while uplifting you to achieve each of your aspirations.
